Terminal Hospitality

With Dr. Von Kamprad in the bag (or supply closest we should say), it's time for our seafaring 47 to motor his way over to hospital island for a little inpatient surgery. This is where the true "specialist" goes to work. It seems that the cult leader, Deewanna Ji, is undergoing heart surgery to install a pacemaker. With the target anesthetized on a table, the Hitman should have easy goings, right? Well, the operating rooms are in the basement of a virtual island fortress, so nothing will necessarily be easy. But like always, Agency has dropped off a special little package in one of the doctor's offices, which should be of some help.

Load-Out

You're likely still packing all the gear handed out back at the market square. In case not, the Shiva temple has a nice supply of firepower for a religious building (on second thought, I guess that does make sense). Inside are AK-47s, a sniper rifle and a Desert Eagle. The guards on the island have made the step up from 9mm pistols to Uzi sub-machineguns, if you can get your hands on one of those.

Walkthrough

You begin the mission on the island shore, behind a large rock outcropping. You can see a single guard patrolling on a large yacht in the harbor, but he really won't be of concern. The other guard outside the hospital premises patrols back and forth along the dock, but when you begin the level he is on the far end, in front of a long row of crates. So, to begin, just run along the dock, hugging the wall until to reach the safety of these crates. Now go into sneak mode. The guard on the boat should be patrolling to your right, while the guard on the other side of the crates will shortly begin his route back to your left.

Make your way to the end of the crates, and when the dock-patrolling guard is too far away to hear your footsteps, run up the grassy hill toward the Shiva temple. Eventually you'll reach a narrow pathway that leads directly to the patio outside the temple. There are two guards that should be of concern here. One stands with his back to the temple, looking out onto the main plaza, while the other patrols around the perimeter of the temple.

When you reach the end of the pathway, check your map, and the patrolling guard should be beginning this route behind the temple, out of sight. In case not, just wait concealed until he does. On the far side of the patio next to the pool is a set of cult member clothing, which you are going to very nonchalantly walk over and change into. Sneak mode isn't even required. The stationary guard has his back turned and is too far away to hear footsteps, while the other guard is busy enjoying the view from behind the temple. So, walk over and snatch the outfit.

At this point, you could run into the temple and grab the extra guns if you'd like, but it's really not necessary. Just walk down the steps to the main courtyard, and head over to the front door, keeping a good distance from other guards. Once inside, the first stop is going to be Dr. Chakran's office, where Agency has left a small greetings package. When you enter, you immediately see two sets of steps leadings to the second floor. Take either the left or right side - it doesn't matter - but it's a good idea to take the lesser occupied of the two. The nurses around this place are wickedly paranoid. At the top of the stairs, head south. Dr. Chakran's office is located in the southwest corner of the building and is marked with the red Agency symbol on your map.

Once you reach the open doorway to his nurse's office, don't walk in just yet. In order to sneak into his main office, you have to wait until the nurse takes a smoke break. So, either watch your map or check at an angle to see when the nurse decides to head out for some fresh air. When she does, that's your cue to slide over to Dr. Chakran's office door and pick the lock. Inside the office, grab the items in Agency's box: night vision and the key to Chakran's adjacent storage room. Since you have the key, might as well have a look inside, eh? Go ahead and change into his doctor's suit, and grab the anesthetic from the desk.

Dressed as Dr. Chakran, it's time to head down to the basement for surgery. The elevator is much easier on 47's old dogs, so let's take that route. Simply walk out of the office, through his nurse's office (if she's back at the desk again, don't worry) and back into the main upstairs lobby. Now head directly north, and try not to let that event-triggered epic Indiana Jones inspirational music ramp up your ego too much - we're not out of the woods yet. Once you reach the elevator (green bouncy thing on the map), call it and take it down to basement level.

Ok, there are a lot of options when it comes to the next phase of 47's mission: killing the cult leader. We're, of course, only going to choose one of them. There are four operating rooms, two of which contain regular patients along with doctors and/or nurses. A third is empty. And the fourth contains the target and two of his bodyguards. The catch is, you don't know who is in what room, and if you walk into the wrong surgery, the doctors recognize you and all hell breaks loose. So, there has to be a way to scope out the situation, and luckily the level provides you with just that.

Check you map, and at the southern end of the basement is a room marked as a point of interest, and it also contains a "ladder" icon. Turn left out of the elevator, and head to this room. Check the map to make sure no patrolling guards are approaching, and go ahead and pick the lock. Inside you'll notice a generator on the right wall, but we'll return to it later. For now, climb the ladder and head into the ventilation ducts. We have to find our target.

Follow the ventilation tube north, until you come to the end of it. As you can see, each operating room has a single vent near its ceiling, and from the ventilation tube, you can peer through each opening into the corresponding room. So, all you have to do is find the room that contains two orange-dressed cult members, and now you know where the target is. (Hint: the room layout on the ventilation shaft map is exactly the same as on the basement map, so just transpose the location down onto basement level.)

Now return to the generator and toggle the power, which turns the lights out. Luckily you have the night vision goggles Agency provided you with, so equip those and head back into the corridor. Directly across the hall is a room labeled "Anesthetist." Walk inside and pick up the scalpel from the desk. Now head directly to the operating room containing the cult leader. Walk over to him on the table and use the menu option to "Kill Cult Leader," and watch as 47 performs his own special brand of heart surgery (hey, he's doing it in the dark!).

When the job is done, exit the room, and then head west down the hallway, exiting into the spiral stairwell. At the bottom of the stairwell is a door that you can pick open and exit to the outside. Now that you're outdoors, grab a breath of fresh air, and make your way up the hill to the main courtyard. The exit point happens to be the same as your start point, but this time around you can take the main staircase. You are a doctor after all. Just watch your step on the way out...
